Medications Request Form

Please note that under Queensland legislation relating to prescription medication for animals, we need your pet to have been assessed within the last 6 months by a veterinarian in order to have a script refilled. If you have any concerns or questions, please feel free to call us on 3801 1744.

Please allow 24 hours’ notice when requesting medications. We will call you as soon as the medication or script is ready to collect.
